INSTRUCTIONS FOR PARENTS: 
ADULT NAME -and- ADULT BIRTHDAY must be entered first.
Child's information will be entered later

IMPORTANT!
1) Orientation Agreements for minors must be signed by a PARENT (or COURT APPOINTED legal guardian).
2) Orientation Agreements signed by a friend, relative or group leader WILL NOT BE ACCEPTED.
